Adapt ADR-1 according to ADR-23 #84

This is the draft for the pull request we will apply to ADR-1 if we accept ADR-23. It's here to illustrate the consequences of ADR-23. Hope that makes things clear.

Clarify that design discussions should go to UIPs
Update file name format of ADR documents
Make it explicit that the result of a discussion should be summarized in the ADR
Clarify that retracted or rejected proposals are not merged
Clarify how to choose an ADR number
Remove trailing whitespace
