Add definitions for how we will use milestones, that they represent releases, and how to assign issues and pull requests to milestones. It also includes some notes about alternatives how to group issues when it's not about release planning.
When this pull request is reviewed and merged I intend to clean up the existing milestones accordingly.
The pull request also adds a README to the directory with a short overview of the documents which are contained there.
