PHP Warnings on initial install

What steps will reproduce the problem?
1. New install of FusionPBX, follow steps on first 2 screens

What is the expected output? What do you see instead?
Login screen after initial setup
Instead screen is empty (just the background image, no login area)

What version of the product are you using? On what operating system?
R1222 on OpenWRT

Please provide any additional information below.

After reloading the default web page there is no apparent problem with 
operation.

I did a clean install with sqlite and didn't see any errors.
