Relay never triggers #2

What steps will reproduce the problem?
1. Start simple EVSE : blinks regularly (2.5 blinks / s), relay does not switch 
ON
2. Plug the J1772 connector to the vehicle (Nissan LEAF 2105)
3. 1 + 2 beeps indicating the vehicle has seen the plug
4. EVSE blinks a little slower, relay does not switch ON
5 Vehicle indicates charging error (3 blue LEDs blinking simultaneously

What is the expected output? What do you see instead?
Expected output xas relay to switch ON and vehicle starting to charge 

What version of the product are you using? On what operating system?
N/A

Please provide any additional information below.
Relay was tested : works fine when switch to the 12V power

Any way to test simple EVSE?

same issue here! Chevy volt!

the relay does not engage.
when i manually connect the relay the car starts to charge!
