Filename field disappears in Jupyter notebook. How to restore it? #471

As shown in the attached, filename field that is supposed to be above tool bars (File, Edit, View, Insert, Cell, Kernel, Widgets, Help) disappeared... How can I restore it? Any help will be very appreciated. Screenshot_jupyternotebook
