Bans not working.. #263

My bans are NOT working! (I have tried to put this up before, so ill do it 
again) I ban someone, they can come back. I do not know why. Also, I go into 
HLSW and ban someone for their ID or IP, and they can STILL come back! Is it a 
problem with my server? I really need help!
