Add debug flag and debug asserts

A debug flag would allow for more rigorous defensive programming practices without having to worry about performance. Suggested asserts:

• [x] no duplicates in lintreq
• [x] linter problems are sorted by Span
• [ ] overlapping regions in lintreq
