Updates to BEAR relevance page

Changes that need to be made to the relevance page of the BEAR flow

• [x] Add a % to the right of the baseline risk input box
• [x] Dynamically create the NNT value using the baseline risk input value
• [x] Change Any further comments? question to Clinical bottom line. Update placeholder text to say What are you going to do in the light of this evidence...
  • [x] will need to run update_questions.exs file on the server once this has been merged to update the question in the database.
