Adds definition of hoisting #8 and declarations/initialisation which supplement the hoisting definition
Makes small edits following @nelsonic's review of the PR #1

I'd like to assign @Brymon to take a look at this PR to see if the definition is clear but I'm unable to, perhaps tagging will help 😊
