Simplify proxy rules. #50

@dboreham I believe that OS X needs different modules to be loaded. Maybe we can isolate that portion of the file into another .conf file to make this consistent with the xbox repo?

I think I missed some of the plot here : What's the reasoning behind there being this file distinct from the "real" (not "testing") file?

@richburdon had asked for an OSX testing setup with proxying. The rule syntax is identical, but the paths and module names are slightly different on OSX. Also, the real file is in the xbox repo. There is probably a good way to make a file with just the rules, stick that somewhere accessible, and then include it, but as-is, we have separate files.

When we talked about simplifying the rules, I started with this to ask for comment, and then would plan on syncing up on the xbox side.

These work for me, although I should note that I have not tested the /app rule as I don't have any apps locally deployed at the moment.
