Add Disabled/Enabled state toggle on Rules page of item sheets

Add a global Enable/Disable state for all rules on a given sheet.

Condition & Effect items should also have a toggle in the Effects Tab Items will have this toggle in the form of an 'Equipped' state (see #640)
