In README.md, the reference to CONTRIBUTING.md is to a template file https://gist.github.com/PurpleBooth/b24679402957c63ec426. We should copy it over, and update the code of conduct enforcement email which is currently "[INSERT EMAIL ADDRESS]", as well as the how to contribute portion.
