documentation still refers to .xhtml, when the default is now .html #20

The documentation needs to be corrected to reflect our change from making standalone .xhtml files. There's just too many compatibility issues involved with trying to deliver standalone .xhtml, so we've switched to a more traditional .html + .js approach.
