Before the tool

[ ] TOS presented in between landing page and tool
[ ] some sort of a landing page with this basic info: http://webapp.e-nable.me/ (new issue soon)

In the tool

[ ] Select type ofo limb difference [for now, a) ABS with no fingers and no thumb or b) no fingers but with thumb]
[ ] Select left/right hand
[ ] Instructions for taking a measurement
[ ] Field for inputing width of residual palm
[ ] Download button
[ ] Email field for zip file
