In the obs advanced settings there is an option for color format which has to be nv12. I'm curious, what is to at the moment and why have you set it to that?
Alternatively you can use the "cpu" version of this plugin which despite its name still mostly runs on the gpu. That might fix it.
I had it set to RGB and switching it to nv12 works!
Had the same Issue happen to me, tried the cpu version of the plugin and still got the same error. my color format is already set to nv12 as well.

There is something else wrong with your setup because it can't even initialize AMF. Maybe make sure you are on recent drivers but other than that I don't know how to debug it more.
Found the problem, it was the drivers. I was using NimeZ drivers and it doesn't install the amf encoder for some reason. Using latest AMD drivers now and works good.
