Based on this paper: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC9797178/ we have interpreted the primary fluence as the diagonal crossbeam profile. I believe the authors of the paper and the original matRad developers physically measured the primary fluence.
In our case, we used a combination of our machine commissioning data as well as measurements made (virtually) in our treatment planning software (Eclipse).

I'm not a physicist so I'm probably not the best person to ask. I believe the crossbeam profile is usually measured with an ion chamber in a water tank. For us, we used the data that was already collected during machine commissioning.
You can also supplement the data if you have the machine commissioned already in a treatment planning system. We wrote a script in Eclipse to grab the data for us.
