fix for PHP 7 warning

Implemented changes: ($stories['completed'] ? Completed : Incomplete) - different in includes\storyblock.php

includes\corefunctions.php - needed because coauthors

Untouched: includes\categories.php includes\categorylist.php includes\characterlist.php includes\javascript.js

includes\seriesblock.php - it uses fixup_newlines()

includes\deletefunctions.php

Fixed includes\reviewform.php
