nutritionSurvey: drop uuid properties #38

uuid is a relic from a previous design choice and is a duplicate of _id (CouchDB's doc ID).

@johanneswilm, @readevalprint, does the backend consume either of these properties?

The uuid is saved on the importer. I am not 100 percent sure, but I think it's from form hub. I'll clarify in the morning.
