Split app-specific components into their own themes #3

Could we have a base eHealth theme, with app-specific overrides in their own theme (such as the icons and nav bars)?

@tlvince I suppose the answer to this is, if the class or mixin is something likely to be used in other additional apps, then it should be committed back to this repo. Otherwise, I'd say just keep it in that specific app.

Maybe some examples of what sort of things you're thinking about adding back upstream?
