Case Data(Vaccination) Date of last Vaccination to be part of case data.

Proposed Solution

Once Vaccination is selected under case data, allow user to input last date of vaccination

07/07/2024: Issue has been resolved for Measles. GHS has been prompted to provide the list of diseases that must be updated with vaccination information. 07/07/2024: Date of last vaccination to be applied to the following case-based forms: - IDSR - Anthrax, Dengue Fever, Plague, Rabies, Smallpox, SARS, - Measles - Yellow Fever @douglasderrick
