Storage Type - Review and Fix #45

I have received reports from both Shrey and Chris Delis that they have had
bad results with choosing either the Mixed or the MYSQL data storage
options.  It appears that only the Lucene option (which uses MYSQL
minimally for resumption tokens) works correctly.   The storage mechanism
options need to be reviewed, the options weighed and then the code and
documentation changed accordingly.

Rev. 107 allows for mysql and mixed storage types (again).  Storage type lucene 
is still preferred, however.
