Improve language detection #35

I'm going to go through languages.json and pygment's lexer determination code to figure out some fixes we should apply.

Mostly fixed. The markdown issue will be difficult to resolve, if at all, until Pygments supports Markdown. Or we could just kill support for MiniD.
