convert questionnaire format to send exchange format to backend and get back spreadsheet

This is currently mocked to give back a dummy spreadsheet

I've created a pull request (28) setting up the framework for the service handling the actual template generation. I'll be away for the next few days, but someone else can continue work on this.

@rdgoite , i'll let you continue the work you started here since you're already back tomorrow. I'll pick up the task next in priority.
