prescriptions for iv antibiotics: new measure suggestion #2384

I have had a brief discussion with an antimicrobial stewardship expert off the back of our new measure on prescribing of hospital only medicines

They think our data is very useful for highlighting potential inappropriate use, unevidenced based use of iv antibiotics in certain clinical situations (e.g. for catheter changes).

I have had a quick look and some iv antibiotics (e.g. Wockhart Amoxicillin) is not hospital only. Another measure to support antimicrobial stewardship might be all iv antibiotics.

This is a reminder to explore a potential measure at a later date.

We have prototyped the measure in a notebook. Some decisions and feedback from users with an interest in antimicrobial stewardship

  1. colistimethate sodium/colistin/colomycin we will exclude as injection is more commonly used via inhaled routes for the treatment of infection in people with cystic fibrosis (CF).

  2. Other injectable preparations used inhaled route for CF and other respiartory conditions: tobramycin, gentamicin etc. I have seen all used via inhaled routes for treatment but we will include these in the measure as a) now specific inhaled preps exist so they should be used in preference as per license where appropriate b) it may not be appropriate for GPs to prescribe.

  3. Should we include just antibacterials? Decided to include all types in chapter 5 i.e. antifungals, antivirals as its probably less likely to be appropriate for a GP to prescribe for clinical or funding reasons.
