Change antibiotic denominator to "all oral antibiotics" #3161

Some of the abx measures have a very specific subset of antibiotics as denominator. It is proposed that these markers are changed to reflect all oral abx in both numerator and denominator, using dm+d coding.

PRs ready for review. Look to merge incrementally in this order:

[x] Broad spectrum % - not associated with STAR-PU changes
[x] Broad spectrum per list size - changing denominator from STAR-PU to list size but STAR-PU not validated for this subset
[x] All antibiotics per list size - changing denominator from STAR-PU to list size - possibly most controversial
[ ] Remove STAR-PU from analyse page
[ ] Remove STAR-PU from FAQs (or better yet - explain why we stopped using them)
