Add instrumentation as separate MD files #23

Add Instruments Quantification of Sci ID (QoS), Qualitative Identity Index (QII), and Instructor Guideline Adherence Rating (IGAR) instruments as individual markdown files with defined purposes and proposed question formats.

@marksherman - the instruments have been completed to the best of my ability - the markdown files are contained in the /instruments folder. Let me know what you think :)
