Heroku Setup: Error handling and debugg pipeline creation

Description: Currently when running Heroku setup

• No prompt to confirm Heroku CLI login
• Nasty error within pipeline creation

AC:

• Confirm user is logged into Heroku CLI
• Debug and handle known errors for end-user experience
• Confirm Heroku Setup passes as expected
