218: API V2: rewritePathsAsURIFragments doesn't work #219

Precompute a mapping of standard JSON Pointers to EMF-ish "custom paths" in calculation of patches in the case they will be needed for notifications with the subscription option for URI-fragment paths.

Fixes #218.
