Review and remove Help -> Report a bug menu item contributed by Mylyn #102

In the current EPP configuration Mylyn contributes a help -> report a bug item that opens an empty dialog. See https://github.com/eclipse-mylyn/org.eclipse.mylyn/issues/338

The dialog is empty because we abandoned bugzilla and did not add this functionality to GitHub connector.

Thank you @mjmeijer and @ruspl-afed - I am closing this now as the dialog is useful again. Because only a subset of projects appear there may be some bugs that start in the wrong project, but the issue of blank dialog is well resolved now.
