Prepare for stable release #229

This is the preparation branch for the stable release and it also incorporates the migration of the npm packages to the "glsp" npm org.

Please make sure that all packages can be built properly. Since this is going to be a stable release we also need to double check that no dependencies are pinned to fluent version tags like "next" or "latest".

Also I'm not sure if we should publish the workflow example in the "@glsp" scope as well

Imho this is not really necessary, as the release is mainly for the projects that want to consume glsp for their own diagram type and not really for demo and testing purposes.
