License information for data #3

It would be helpful to include data licenses in the metadata to make sharing easier.

@sebaum For the data sources where I could find terms of use, UNICEF and ProMED have restrictions on data reuse. We should definitely reach out to both groups.

A number of the manually acquired sources do not have links to the data source so I could not verify the terms of use for that data. Here is a summary table. The updated metadata is on the enhancement/TOU_in_metadata branch.
