Implement caller identification on client

Implement caller identification, as per the advanced spec.

Client requirements:

• HELLO.Details.roles.caller.features.caller_identification|bool := true
• HELLO.Details.roles.callee.features.caller_identification|bool := true
• CALL.Options.disclose_me|bool := true
• REGISTER.Options.disclose_caller|bool := true
• Receive INVOCATION.Details.caller
• Handle wamp.error.disclose_me.not_allowed

There is no special treatment required by the client.
