Email the group (our metadata email probably, whoever we ID at epscor, the documenter and all of the identified PI/advisors from the form) if one of the following is true:
requesting CC-BY license
requesting embargo extension (but not if just picking a date from the checkbox)
The embargo one might need to be reviewed - it's a little unclear if we need agreement for any embargo selected or just if there's an extension requested. There are certain expectations based on different kinds of data so a 12mo embargo for one might not be acceptable for another. I guess be prepared for dealing with the embargo email if requesting extension OR embargo time > 0.
We'll also need to come up with a canned email, starting with a subject like "Important! {Embargo | License} Requested. Action Required."
