I believe shift selecting should add while ctrl/meta should toggle. (Check this: maybe all should toggle?)
Illustrator and Inkscape only support shift-clicking, as toggle (they treat control-clicking as different). File Explorer treats shift-clicking as very different (selecting an interval, which doesn't make sense here), and control-clicking as toggle.
Summary: No one seems to support current "add-only" mode. So I think all should toggle. We can remove ctrl/meta-click if we need it for something else later...
