Enhance all default answer fields.

This is an improvement on an existing feature.

Answer fields in v1.5.3 are much better than they used to be, but they are 
still a little clunky. By version 1.7, answer fields should have been enhanced. 
A wiki page has been set up (AnswerFieldsEnhancement) to collect any ideas for 
answer field enhancement.

This relates to Issue 51 which suggests all answer fields look the same in quiz 
and edit modes apart from in edit mode the addition of a small spanner button 
that shows a popup with all the answer field settings in it. This allows for 
more abstract/advanced settings to be included. It also makes the question 
editor more WYSIWYG as the options are hidden away.
