Begin wiring up enclave proxying #56

This does the basics to wire up ingress and egress proxying. This led me to a pretty significant refactor of how running an enclave works, and I'd like to do more still, but this seems like an OK cutoff point for a PR.

More work is needed to:

Improve the UX around TLS setup (and maybe make it optional)
Further improve enclave log output
Support the Nitro Enclaves console in debug mode
