As per the mockup specified, includes Agency Office Code and Sub-Primer num. Includes

• Option 3: Adding the first code of Agency Office Code will autopopulate the Department name

@ambergman can you confirm that agency office code & sub-primer num aren't required to be able to submit the form? (because if you submit the department name, the agency code won't fill for you.)

I screwed up the commit history by accidentally pushing changes meant for datarescue to this repo, Can we just merge this branch, or else revert the range of commits from my last push, and then merge this?
