Add DSL

This PR adds two DSL macro, index/0 and form/0,1 (along with field).

Todo:

• [] Type checking, error on compile time if unknown type (?)
• [x] Verify that it works nicely with things like Ecto.Enum (and assocs/embeds)
• [] Possibly implement belongs_to to make association explicit?
