icd_history: Allow for custom transitions #7

The user should be allowed to specify a data.frame containing custom transitions to augment the DIMDI rules. This is useful when a transition is not formally correct but appropriate in the context of a given project, where an expected change in coding behaviour occurs due to the introduction of a new diagnosis or even due to "rightcoding" initiatives.

To check: Can transitions be one-to-many?
