fix: update description according to the review comment

According to the review comments in https://github.com/obsidianmd/obsidian-releases/pull/1851#issuecomment-1533832563, the following issues need to be fixed.

• update description according to Obsidian guidelines
• remove command which is not in this plugin scope, maybe create another new plugin
• use plugin app instead of window.app which might be removed in the future
