Prologues and Epilogues for Questions and Groups #24

Sometimes we want text in-between questions (or groups), they may not be part of a question itself. This can be something like directions for the next few questions (which can also be done with a text-only question), or styling things. For example, a group can have an epilogue that is a linebreak.

Not all output formats will be able to support these. We should either error or warn on those.

I decided to close this one. With support for formatting hints, this is less necessary.
