writing comment about a cualification shows popup saying [MAIN CONTENT GOES HERE - Nu0nMi0D7F]

Viewing an answer of a student , and clicking to comment the answer he selects, appears a new popup windows, where you could write the comment, but it shows a blank page saying "[MAIN CONTENT GOES HERE - Nu0nMi0D7F]". The url of the page of cualification is: http://www.mydomain.com/mod/quiz/review.php?attempt=10084
