Improve Summary #14

New Features:

render_diff now can now add an summary table to the top of HTML pages (enabled by default).
The 'print' method now defaults to showing only the first and last 6 lines instead of the entire patch.

Internal Changes:

Upstream changes to daff paulfitz/daff#88 that handle summarizing changes allow our code to be much cleaner.
Minor changes to test code.
The default value for diff_data argument count_like_a_spreadsheet changed to match daff.js library default. (Do we want this?)

Bug fixes:

patch_data and merge_data now handle diffs that include additional meta-information columns.
