Switch extension for otherId in Partial Results #6

Instead of using the BALP extension for otherId (which is in a next version of BALP not valid in OperationOutcome), john suggests to use the http://hl7.org/fhir/extensions/StructureDefinition-artifact-identifier.html extension. see background in https://github.com/IHE/ITI.BasicAudit/pull/77
