PIXmPatientFeed profile can't be used for PIXm/PDQm responses #115

The slice cannot be with PIXmPatientFeed because we could have patients without a local id.

The same applies to PIXmOutParameter with the reference to PIXmPatientFeed

Need to create a new profile where localId is not 1 and PIXmPatientFeed can derive from it.
