Rename "CH EPR mHealth" in "CH EPR FHIR" #160

Not renamed in this PR:

The concepts of "mHealth App" and "mHealth API"
The concept of "generic mHealth option"

Out of scope of this PR:

Rename the repository
Update the feedback page
Update the fhir.ch page

Also rename "mHealth App" to "EPR App" and "mHealth API" to "EPR API" (App being a web portal from the community or a primary system). Remove "mHealth" from "other mHealth profiles". Rename "Generic mHealth option" to "Generic EPR API option". Rename file too.
