Replace "Amendment mHealth of Annex 5" with references to the right section in the IG.

Index: Add section "1.2.2 national integration profiles" (before "Related profiles, actors and transactions"), add PPQm.

PPQm: fix version of CH-Core and remove PPQm

Transform three ch-epr-ppqm-policy-rule-* into dedicated value sets.

Mappings: delete section "Mapping between CH:PPQm and CH:PPQ Messages". Try to move the rest to mapping tab of "CH PPQm Consent" profile.
