MHD: Require EPR-SPID as a query patient.identifier

Two variations:

1. MHD Endpoint just using the EPR-SPID (we do not need to specify how EPR-SPID to XAD-PID translation works)
2. MHD Endpoint would accept also XAD-PID for query, the endpoint would need to translate (either from JWT or through PIXm) form EPR-SPID to XAD PID)

Rational: prose to use variation 1 because in CH ATC and for the revised revision an single patient identifier is proposed (every single document request is its nature cross community due to is federated architecture of the EPR)

TODO:

• adjust MHD ITI-67 (and more)
• check compliance to MHD
• check other spec implications
