Add ITI-78 / Mobile Demographics Query to transactions #52

Mobile Demographics Query (ITI-78) could be added to the transactions.

This would allow to query for Patients using Demographic Data (when none of the identifiers is known).

A possible use case could be a mobile app for first responders, that would allow to get access to a patient's EPR (respectively the files visible in the scope of the first responder authenticated as a health professional). In this use case, the identifiers are probably not known.
