IHE Change proposals to the mCSD profile

• ITI-90: On _lastUpdated, prefixes gt, lt, sa and eb have no DSML2-equivalent, but they are SHALL support. -> SHOULD support. They are excluded from this PR. Reported in https://github.com/IHE/ITI.mCSD/issues/143.
• ITI-91: the transaction shall be implemented, but its use-case is much narrower than ITI-90. Not the same users, probably. -> SHOULD support. It is excluded from this PR. Reported in https://github.com/IHE/ITI.mCSD/issues/145.
• ITI-90: Missing rest.resource.interaction.code = #read in CapabilityStatements (corresponding to Retrieve Care Services Resource). Reported in https://github.com/IHE/ITI.mCSD/issues/144.
